How Dental Implants Work: Step-by-Step

Dr. Aykut Gürel·

What Is the Dental Implant Procedure?

Dental implant treatment replaces missing tooth roots with biocompatible titanium posts that integrate with the jawbone and support a natural-looking crown, bridge, or full-arch prosthesis. Published clinical data show 10-year success rates of 95-98% when treatment is planned and executed by an experienced oral and maxillofacial surgeon. At our clinic in Atasehir, Istanbul, the procedure follows a four-stage protocol designed for predictability and patient comfort.

Stage 1: Examination and Digital Planning

The first appointment determines the success of every step that follows. A comprehensive evaluation includes:

  • 3D Cone Beam CT (CBCT) scan for millimetric imaging of the jawbone
  • Bone density and volume analysis
  • Mapping of the inferior alveolar nerve canal and maxillary sinus boundaries
  • Digital planning to determine the exact position, angle, and depth of each implant

This stage typically takes 30-45 minutes. The CBCT scan itself is painless and completed within seconds. Treatment options, expected duration, and cost are discussed at the end of this visit. Detailed digital planning is covered in our 3D imaging guide.

What if there is not enough bone?

When the CBCT scan reveals insufficient bone, the path forward depends on the severity:

  • Mild deficiency: Bone graft placed simultaneously with the implant
  • Posterior upper jaw deficiency: Sinus lift before or with implant placement

Stage 2: Surgical Implant Placement

Implant placement is performed under local anesthesia. Patients feel pressure but no pain. Sedation is available for anxious patients or longer cases.

Guided Surgery for Precision

Our clinic uses guided implant surgery. A patient-specific surgical guide—designed from the CBCT data—directs each implant to its planned position with millimetric accuracy. Benefits include:

  • Shorter surgical time
  • Reduced soft-tissue trauma
  • Improved placement accuracy
  • Less post-operative swelling and discomfort

In suitable cases, flapless (sutureless) surgery is performed. A small access point is made through the gum without raising a flap, and the implant is placed through this opening. Healing is faster and patient comfort is markedly higher.

A single implant placement typically takes 15-30 minutes. Multiple implants take proportionally longer.

Same-Day Implants (Immediate Placement)

In selected cases, the tooth is extracted and the implant is placed in the same session. This immediate implant approach shortens overall treatment time and preserves bone volume around the extraction site.

Stage 3: Osseointegration (Bone Fusion)

After surgery, the titanium implant gradually fuses with the jawbone. This biological process—osseointegration—forms the foundation of long-term success.

A temporary prosthesis can be worn during this period to restore aesthetics and chewing. Detailed expectations are covered in our recovery timeline guide.

Stage 4: Final Prosthesis

Once osseointegration is confirmed, a digital or conventional impression is taken. The final crown, bridge, or full-arch prosthesis is fabricated from zirconia, lithium disilicate (e.max), or porcelain-fused-to-metal, depending on location and aesthetic requirements. Each restoration is custom-shaded to match adjacent natural teeth.

Material differences and brand options are detailed in our implant brands comparison guide.

Recovery Essentials

Mild swelling, bruising, and tenderness during the first days are normal. Standard post-operative recommendations include:

  • First 48-72 hours: Cold compress on the cheek (20 minutes on, 20 minutes off)
  • Diet: Soft, lukewarm foods during the first week; avoid hot, spicy, hard items
  • Smoking: Avoid for at least 2 weeks before and after surgery—nicotine impairs osseointegration
  • Medications: Complete the prescribed antibiotic and pain medication courses
  • Oral hygiene: Use the prescribed mouthwash gently for the first few days; avoid brushing the surgical site directly
  • Follow-ups: Routine check-ups at 1 week, 1 month, and 3 months

A complete post-op protocol is described in our recovery timeline guide.

For International Patients

Patients traveling to Istanbul for implant treatment typically plan a 5-7 day stay for single or multiple implant placement. A representative schedule:

  • Day 1: CBCT scan, clinical examination, digital planning
  • Day 2-3: Implant surgery and immediate post-operative review
  • Day 5-7: Final follow-up and travel clearance

If you have a recent CBCT scan in DICOM format, sending it in advance allows preliminary digital case planning before your trip. The final prosthesis is fabricated 3-6 months later, often requiring a brief return visit of 2-3 days.

Frequently Asked Questions

Is implant surgery painful?

The procedure is performed under local anesthesia, so no pain is felt during surgery. Post-operative discomfort lasts 1-3 days and is managed with standard pain medication. Most patients describe the experience as more comfortable than they expected.

How long does the entire treatment take?

Single implant cases: 15-30 minutes for placement, 3-6 months total including osseointegration. Bone grafting or sinus lifting can extend the timeline to 6-12 months. Same-day immediate implants are sometimes possible in suitable cases.

What is the difference between guided and freehand surgery?

In freehand surgery, implant positioning depends on the surgeon's manual technique. In guided surgery, a 3D-printed surgical guide ensures the implant is placed exactly where digitally planned. This reduces tissue trauma, shortens surgery time, and adds a safety margin near nerve canals or sinuses.

When can I eat normally?

Soft foods for the first 3-5 days. Most patients return to normal eating by the end of the first week. A temporary prosthesis—if placed—supports light to moderate chewing. Once the final prosthesis is delivered, full diet is restored.

Can any dentist place dental implants?

Any licensed dentist may legally perform implant surgery, but outcomes correlate strongly with the surgeon's training and experience. For complex cases—bone deficiency, sinus lifting, full-arch rehabilitation—an oral and maxillofacial surgeon offers specialized expertise.
